• Assess patient needs in home and community settings
• Deliver therapeutic nursing services to clients
• Coordinate resources and set care goals
• Evaluate clinical outcomes with the health care team
• Mentor students and participate in staff development Requirements:
• Completion of accredited nursing education program
• Registration with the College of Registered Nurses of Alberta
• Current Basic Cardiac Life Support certification
• Palliative care and geriatric nursing experience preferred
• Minimum of 2 years case management experience Utilize your nursing skills to provide holistic, patient-centered care within the Palliative Care team at Assisted Living Alberta.
