Overview The Supports for Community Living Services position is pivotal in enhancing the quality of life for individuals with disabilities, empowering them to lead fulfilling, independent lives within their communities. In this rewarding role, the Support Worker provides personalized assistance, guiding individuals in pursuing personal goals and aspirations. By promoting overall well-being, the Support Worker helps individuals build confidence and skills needed for meaningful participation in community activities and experience personal growth and fulfillment.

Key Responsibilities

  • Meet with Individuals and provide information about brain injury and brain injury services.
  • Assess Individual’s community support needs to enable them to live, work and participate in the community.
  • Work with Individuals, their families, and community agencies to develop service plans.
  • Refer Individuals to Supports for Community Living (SCL) (if the Grant Recipient does not provide this service) where Individuals have an identified need and capacity to learn independent living skills.
  • Assist the Individual and/or their family to apply for benefits and services.
  • Follow up with the Individual and their family as required to help ensure services provided meet the needs and wishes of the individuals impacted.
  • Establish and maintain working relationships with health and community supports professionals including appropriate Service Delivery Partners.
  • Promote regional information sharing and training for Individuals, families, professionals, staff, and community.
  • Collaborate with other Grant Recipients from across the province to coordinate their services and ensure a smooth transition within and between regions.
  • Collaborate with other Grant recipients from across the province and participate in provincial brain injury forums to ensure services are coordinated provincially.
  • Actively promote ABII services in the region to Individuals and their families/support networks.
  • Services will also be offered to the Individual’s family and support network as required to meet the Individual’s needs.
  • Provide short-term training and support to Individuals who have the potential to benefit from training to increase their independence.
  • Provide support for the Individual’s support network, helping them to develop support skills that contribute to long term solutions.
  • Work with Individuals in the community to develop individual support networks.
  • Allow Individuals to demonstrate appropriate skills and behaviors necessary for functioning independently at home and in the community.

Job Types: Part-time, Freelance, Permanent

Pay: $21.12-$26.84 per hour

Expected hours: 30 – 35 per week
